Here is a quick overview of the application process:

  1. Register your jurisdiction: Each jurisdiction must first register online to participate. This registration process will include submitting information such as your jurisdiction name, address, Congressional District, jurisdiction population, the number of full and part-time officers and banking information.
  2. Prepare and Submit Application: After completing and submitting the registration, you will need to fill out the application, which allows you to select the number, style, cost, threat level, and distributor (where applicable) from which you are purchasing.
  3. Report Receipts: After submitting both registration and application and you have received approval on your application, you can submit the "Request for Payment" form. Here you will report exactly what the vests cost, how many vests were purchased and received.
  4. Request Payment: You will also need to review your banking information and submit the "Request for Payment" to have the funds transferred to the account that you provided in your Registration and Application

NYS Soft Body Armor Program


New York State's Soft Body Armor Program
In on ongoing effort to help defer the costs of life-saving body armor for municipal police agencies, the Division of Criminal Justice Services (DCJS) administers, pursuant to Executive Law, article 35, section 837-d, the Bullet Proof Vest Reimbursement Program. This is a open-ended reimbursement program: police agencies may be fully reimbursed for the purchase of vests any time during the fiscal year. However, this program covers only vests for new hires of police officer status. It does not provide reimbursements for replacement vests or funds for newly hired peace officers.

Recently, the Federal government developed its own soft body armor program as an important resource to the State's law enforcement community. Because of a statutory requirement in the New York State program (subsection 4 of the Executive Law) , all police agencies must apply for any available Federal funding before they may be eligible for State reimbursement. Unlike the New York program, the Federal program will provide 50 percent reimbursement for newly hired officers and can be used to partially reimburse localities for the costs associated with replacing old or ill-fitting vests. Additionally, this program benefits both police and peace officers. While the Federal program limits the amount of reimbursement to half of the vest cost, it actually provides more funding as it covers a wider number of criminal justice agencies and a wider population of officers.
The Federal program's application involves a fifteen-week period during which time municipalities may apply for funds. Such requests can only be made electronically on the program's website: http://vests.ojp.gov .
Once Federal funding is either acquired or denied, police agencies then have the option of applying for New York State funds. It is possible for police agencies to be reimbursed for 50 percent of the vests for new hires under the Federal program, and then to be reimbursed for the other 50 percent from the State program.

To access and use these forms, use the links below to select the form you need in the format you prefer. Copy the form to your PC hard drive using the File/Save As commands. We hope that making these forms more readily available will will assist you in the application process. Please note that, due to the need for original signatures these forms cannot be e-mailed to DCJS, but must be sent through regular mail. Also, vouchers area available online on the Grantee Forms page.


All forms are required to apply for reimbursement for Zylon vest replacement. For other reimbursement applications, omit the Zylon certification form.

SOFT BODY ARMOR VEST REIMBURSEMENT PROGRAM - CHECKLIST 

Please follow the listed instructions and complete the enclosed forms before submitting for reimbursement.  For your convenience, the instructions are in a checklist format.

  • Insure your department's eligibility by referring to the attached General Information sheet(s).
  • Purchase only vests which conform to specifications as outlined in the attached material.
  • Include a copy of the Vendor's (dealer's) invoice marked "PAID".
  • Include the completed Soft Body Armor Assignment Log (Pg. 1 and 2).
  • Complete the Certification Re: Soft Body Armor (SBA) Threat Level Protection form (Page 3 of 3).
  • Complete Federal Certification Form.
  • Return completed Vendor's (dealer's) Statement with your reimbursement request.
  • Complete and return "Soft Body Armor Reimbursement Application," DCJS-2025, in duplicate.
  • Complete and return "State of New York - State Aid Voucher" included in your packet.

INSTRUCTIONS FOR PREPARING STATE AID VOUCHER

Complete on typewriter word processor or with pen and ink. Submit OSC and Agency copies to the State Agency administering the program.

1.  Insert name of State Agency to whom claim is being submitted.

2.  Enter your 12 digit Municipality Code.  The first 9 digits are entered in the "Payee I.D." block. The last 3 digits are entered in the first 3 positions of the "Payee Additional" block.

3.  Enter your Zip Code.

4.  Enter the title of the fiscal officer, the municipality name and address as you wish it to appear on the check.

5.  Enter in Rev/Inv. No. block, the information you will need in order to identify this payment. In no instance should this reference exceed 20 characters including spaces, commas, etc.  The check stub issued to you will contain the information you furnish in this block, along with reference/invoice date, if entered in the block below Rev / Inv. No.

6.  Enter in body of voucher all pertinent information required by the specific column heading or any other information required to support the claim.  Duly authorized signature must be shown on supporting City of County vouchers.

7.  Enter in appropriate block the State Aid Program or applicable statute under which claim is authorized.

8.  Complete Payee Certification. Signature and title of the municipal officer, or duly authorized representative, must appear in the space provided. Sign declaration in ink - No Rubber Stamp.

Submit directly to the State Agency which has charge of program.

If the space on this form is insufficient, start your claim on "Continuation Sheet", Form AC 1172, and bring final total forward to this form.
